For example, organising a free session for the patients every evening where they would be able to interact with other patients as well, this would help them to increase their socialising skills and explore themselves at their own pace (Gujral et al., 2020). On the other hand, it has been observed that while adopting the mentioned care approach, it is important to motivate the patients, communicate with them effectively, providing them advice if they expect. Communicating beyond professional boundaries might not be possible for the health and social care practitioner as this might be against the standards of professional communication. However, risk taking is also about acquiring more information regarding the patients; this includes knowing and understanding preferences and needs of the patients. After this, it would become easier to serve the patients depending upon their needs and requirements and also help them to recognise their own value.

Relating to my job role within Redbridge care home I have observed that empowerment-based strategies are highly suitable for dealing with patients with diabetes. It is important to identify main 3 guiding principles which are "role, targets and flexibility", along with this, it is important to formulate personal self-management plan, conduct experiment and evaluate the plan. The main reason of adopting this strategy is to promote quality of life to the patients. In this, lifestyle and behavioural patterns both are taken care of. New model of thinking can be implemented which is effective for recognising whether the patients are in control and would be able to support self-management plan (Nazir et al., 2018).

"Empowerment and involvement" both are considered to be appropriately suitable to implement for the benefit of the patients and increases their involvement into their daily activities. After implementing the mentioned strategy, I have observed that involvement of the patients have increased and this has contributed towards health promotion. Patient empowerment signifies "patient-centred and collaborative approach" which can be considered as an intervention or strategy to develop a vision regarding making decisions for the benefit of the patients (Mogre et al., 2017).

In response to the above study, it can be concluded that it is important for health and social care practitioners to adopt empowerment based strategies which would help them to analyse complex needs of the patients and work upon it. It has been described about the benefits of involvement of multidisciplinary team in treating diabetic patients within the chosen healthcare setting Redbridge. Thus, appropriate recommendation has also been provided in order to analyse the appropriate method and strategy of handling patients with diabetes.
